局面状類乾癬（きょくめんじょうるいかんせん）
局面状類乾癬とは
局面状類乾癬は、乾癬に似た表面が白っぽくガサガサした紅斑が多発する疾患です。
皮疹は長期間続くことが多いです。
かゆみなどの症状はないことが多いです。
好発部位は体などの非露光部で、好発年齢は中高年者です。
ひとつの皮疹の大きさが５ｃｍ以下を小局面型、５ｃｍ以上を大局面型と分類します。小局面型は良性疾患と考えられていますが、大局面型は10%ほどの割合で菌状息肉症（悪性リンパ腫の一種）へ移行すると言われています。
それぞれの患者さんが菌状息肉症を発症するかどうかについて予測する方法は残念ながらありません。
診断
詳しい問診と皮疹の性状によりこの疾患を疑います。
必要に応じて皮膚の一部を採取する皮膚生検を行って診断を確定します。
治療
まずはステロイドなどの外用薬により治療を開始します。
外用のみで十分な効果が得られなかった場合は、ナローバンドUVBなどの光線療法を併用します。
菌状息肉症への移行を疑う所見がないか経過観察していきます。
日常生活では、日光浴が皮疹の改善につながることがありますので、適度な日光浴をお勧めします。

Parapsoriasis en plaque
What is parapsoriasis en plaque?
Parapsoriasis en plaque is a disease with frequent psoriasis-like, whitish and rough erythema. The rash often lasts for a long time. Often there are no symptoms such as itching. The predominant part is the non-exposed part such as the body, and the predominant age is middle-aged and elderly. A rash with a size of 5 cm or less is classified as a small plaque type, and a rash of 5 cm or more is classified as a large plaque type. The small plaque type is considered to be a benign disease, but the large plaque type is said to progress to mycosis fungoides (a type of malignant lymphoma) at a rate of about 10%. Unfortunately, there is no way to predict whether each patient will develop mycosis fungoides.
Diagnosis
We suspect this disease by detailed interviews and the nature of the rash. A skin biopsy is done as needed to confirm the diagnosis.
Treatment
First, start treatment with external medicine such as steroids. If the effect is not sufficient with external use alone, use phototherapy such as narrow band UVB. We will follow up for any findings that suggest the transition to mycosis fungoides.
In daily life, sunbathing may improve the rash, so it is recommended to sunbathe moderately.

滴状類乾癬（てきじょうるいかんせん）：慢性苔癬状粃糠疹（まんせいたいせんじょうひこうしん）
滴状類乾癬とは
滴状類乾癬は、局面状類乾癬と苔癬状粃糠疹に大きく分類されます。
一般的に滴状類乾癬というと慢性苔癬状粃糠疹という病気を指すことが多いです。
慢性苔癬状粃糠疹はすべての年齢で発症しますが、特に青年期に多いです。フケのようなものが付着した赤い皮疹が体や四肢に多発します。
皮疹が長く続くと、色素沈着や色素脱失など様々な皮疹が混在するようになります。
かゆみはない場合が多く、あっても軽度の場合が多いです。
症状は数週間～数年にわたって出没を繰り返しながら続きますが、命にかかわることはありません。
原因ははっきりしていませんが、細菌やウイルス感染に引き続いて発症するとも言われています。
なお、局面状類乾癬については、別途記載します。
診断
皮膚の見た目から診断します。必要に応じて、皮膚の一部を採取する皮膚生検などを施行します。
治療
ステロイドの塗り薬が第一選択になります。良性の疾患であり、無治療でも自然軽快する患者さんもいます。
必要に応じて、ナローバンドUVBなどの光線療法やテトラサイクリン系抗菌薬投与などを行う場合もあります。
日常生活では、かゆみがあってもなるべく掻破しないように気を付けます。
治癒するまで長期間かかることが多いですが、命にかかわることはないため気長に治療を続けていきます。
Guttate parapsoriasis: Pityriasis lichenoides chronica
What is guttate parapsoriasis?
Guttate parapsoriasis is broadly divided into plaque psoriasis and lichen planus pityriasis. Generally speaking, guttate parapsoriasis often refers to a disease called pityriasis lichenoides chronica.
Pityriasis lichenoides chronica develops at all ages, but is especially common in adolescents. Red rashes with dandruff often occur on the body and limbs. If the rash lasts for a long time, various rashes such as pigmentation and depigmentation will be mixed. Itching is often absent, and even if it is, it is mild. Symptoms continue to appear and disappear over weeks to years, but they are not life-threatening. The cause is not clear, but it is said to follow bacterial and viral infections.
Diagnosis
Diagnosis is based on the appearance of the skin. If necessary, a skin biopsy will be performed.
Treatment
Steroid ointments are the first choice. It is a benign disease, and some patients recover spontaneously without treatment. If necessary, phototherapy such as narrowband UVB or tetracycline antibiotics may be given.
In daily life, be careful not to scratch it even if it is itchy. It often takes a long time to heal, but it is not life-threatening, so please continue the treatment patiently.
