痒疹（ようしん）
痒疹とは
痒疹とは、強いかゆみのあるブツブツです。
最初は小さなブツブツとして現れて、掻いているうちにガサガサした浅黒い皮膚になっていきます。
痒疹は長い期間続き、少しずつ数も増えていきます。
なかなか治りません。
アトピー性皮膚炎でも、ときに同じような皮疹がみられます。
痒疹は急性期、亜急性期、慢性期に分けられます。
急性期痒疹は幼小児の時にみられます。
手足や体にブツブツがたくさん出るもので、小児ストロフルスとも呼ばれます。
虫刺され後に生じることが多く、虫刺されにより生じた過敏反応だと考えられています。
亜急性期痒疹は、大人に蕁麻疹のようなブツブツが手足や体にたくさん生じます。
掻くことで傷あとや軽い出血を伴います。
慢性期痒疹は、ブツブツが長い間続いているもので、結節性痒疹と多形慢性痒疹に分けられます。
どちらも皮膚がガサガサして厚くなり、色も浅黒くなります。
お年寄りに多くみられます。
診断
皮膚の状態をみて診断します。
ときに悪性リンパ腫などの病気が隠れている場合もあるため、必要に応じて皮膚の一部を採取する皮膚生検という検査や血液検査をします。
治療
基本的には、アトピー性皮膚炎のような慢性の炎症反応と考えられていますので、アトピー性皮膚炎に準じた治療をします。
具体的には、塗り薬や飲み薬、スキンケア、悪化因子を避けることなどを行います。
塗り薬は、十分な強さの塗り薬を使います。ブツブツが治まっても外用を完全にはやめません。
少しずつ塗る頻度などを減らしながら続けていきます。
なかなか治らない所にはステロイドのテープ剤を使うこともあります。
また、保湿剤、ステロイドの注射、紫外線療法を併せることもあります。
飲み薬としては、痒みが強い場合に、短い期間ステロイドや免疫抑制剤を使う場合があります。

Prurigo
What is prurigo?
Prurigo is a highly itchy rash. It first appears as a small rash, and as it is scratched, it becomes rough and dark skin. Prurigo lasts for a long time and gradually increases in number. It's hard to heal. Atopic dermatitis sometimes has a similar rash.
Prurigo is divided into acute, subacute, and chronic phases.
Acute prurigo is seen in young children. It causes a lot of papules on the limbs and body and is also called pediatric strophulus. It often occurs after an insect bite and is thought to be a hypersensitivity reaction caused by an insect bite.
Subacute prurigo causes a lot of urticaria-like rashes on the limbs and body in adults. Scratching causes scarring and slight bleeding.
Chronic prurigo is a long-standing rash that can be divided into prurigo nodularis and prurigo multiformis chronica. In both cases, the skin becomes rough and thick, and the color becomes dark. It is often seen in the elderly.
Diagnosis
Diagnosis is made by looking at the condition of the skin. Occasionally, a disease such as malignant lymphoma may be hidden, so a skin biopsy or blood test may be done as needed.
Treatment
Basically, it is considered to be a chronic inflammatory reaction such as atopic dermatitis, so treatment is based on atopic dermatitis. Specifically, we will do things such as topical and oral medicines, skin care, and avoiding aggravating factors.
Use an ointment that is strong enough. Even if the rash subsides, you will not stop topical application altogether. Continue while reducing the frequency of application little by little. In places where it is difficult to heal, steroid tapes may be used.
Moisturizers, steroid injections, and UV light therapy may also be combined.
As a medicine to be taken, steroids and immunosuppressants may be used for a short period of time when itching is strong.

色素性痒疹（しきそせいようしん）
色素性痒疹とは
体に強いかゆみのあるブツブツが繰り返し出てくる病気です。
ブツブツの赤みが治まったあと、網目状で薄茶色の痕が残ります。
１０～２０代の女性に多いです。
よく生じる体の部位は、うなじ、背中、肩、胸です。
この病気は過度のダイエットや急激な体重減少、糖尿病の発症などを誘因として生じることが多いです。
診断
よく生じる部位に典型的な皮疹が生じていることを視診し疑います。
また、過度のダイエットや絶食、糖尿病の有無を確認します。
必要に応じて、血液検査・尿検査を行ってケトン体の量を調べたり、皮膚生検（皮膚の一部を採取する検査）をします。
治療
テトラサイクリン系抗菌薬の飲み薬がよく効きます。
糖尿病があればその治療を始めます。
ダイエット、食欲不振、甘い飲み物の多飲、不規則な生活が原因と考えられる場合は改善します。
Prurigo pigmentosa
What is prurigo pigmentosa?
It is a disease in which a strong itchy rash appears repeatedly on the body. After the redness of the rash has subsided, a mesh-like, light brown mark remains. Mostly women in their teens and 20s. Common body parts are the nape, back, shoulders, and chest.
The disease is often triggered by excessive dieting, rapid weight loss, and the onset of diabetes.
Diagnosis
Inspect and suspect that a typical rash has occurred in a common area. Also, check for excessive diet, fasting, and diabetes. If necessary, a blood test and urine test are performed to check the amount of ketone bodies, and a skin biopsy is performed.
Treatment
Tetracycline antibiotics work well. If you have diabetes, start treating it.
If we suspect that your diet, loss of appetite, polydipsia of sweet drinks, or irregular lifestyle is the cause, improve.

多型慢性痒疹（たけいまんせいようしん）
多型慢性痒疹とは
最初は痒みの強い蕁麻疹のように生じて、だんだんと薄茶色のガサガサした肌になっていきます。
湿疹は長い間続きます。
年齢的には中年以降が多く、お腹まわり、腰やお尻などに生じやすいです。
診断
塗り薬の治療薬を始めてもなかなか治らない場合に、この病気を疑います。
皮膚リンパ腫というがん、薬による湿疹などの可能性もあるため、血液検査や皮膚生検という検査を行う場合もあります。
また、胃や腸など内臓の癌が原因になっている場合もあります。
治療
塗り薬と飲み薬を組み合わせて治療します。
塗り薬は、体の部位や湿疹の状態によって強さを選んでいきます。
保湿剤も併せて使っていきます。
飲み薬は、痒みに対して抗アレルギー薬を使ったり、特定の抗菌薬を使ったり、湿疹がとても悪い場合は免疫抑制薬を使う場合もあります。
紫外線をあてる光線療法も効果が高く、塗り薬・飲み薬と併せて行います。
日常生活では、ナイロンタオルでゴシゴシ擦るような強い刺激を避けるなど、悪化因子になる生活習慣を改善します。
Prurigo multiformis chronica
What is prurigo multiformis chronica?
At first, it develops like urticaria with strong itching, and gradually becomes light brown and rough skin. Eczema lasts for a long time. Eczema lasts for a long time. It often occurs after middle age, and it tends to occur around the abdomen, hips and buttocks.
Diagnosis
We suspect this illness if it is difficult to get rid of it even after starting an ointment treatment. Because there is a possibility of cancer called cutaneous lymphoma and eczema caused by drugs, a blood test and skin biopsy may be performed. It may also be caused by cancer of internal organs such as the stomach or intestines.
Treatment
It is treated with a combination of ointment and oral medicine.
The strength of the ointment is selected according to the part of the body and the condition of eczema. We will also use a moisturizer.
Oral medicines may include anti-allergic drugs for itching, certain antibacterial drugs, and immunosuppressive drugs if the eczema is very bad. Phototherapy that exposes to ultraviolet rays is also highly effective, and is performed in combination with topical and oral medicines.
In daily life, it is necessary to improve lifestyle habits that are aggravating factors, such as avoiding the irritation of rubbing strongly with a nylon towel.
