口唇の疾患
口唇炎（こうしんえん）
口唇炎とは
口唇炎とは、その名のとおり、くちびるに炎症が起こる疾患です。
原因は様々ですが、主に次のようなものがあります。
① 接触皮膚炎
くちびるに外的刺激物が付着することによって生じます。
いわゆる「かぶれ」です。
かゆみを伴うことが多いです。
原因としては、リップクリームや口紅、歯磨き粉、化粧品であることが多いです。
問診により原因物質を推測し、その使用を中止するようにします。
必要に応じて、原因を特定するためにパッチテストを施行します。
治療としては、赤みが強いときはステロイドの塗り薬を使います。
日ごろのケアとしては、ワセリンを外用してくちびるを保護します。
② 単純ヘルペス
口唇にピリピリした痛みを伴う小水疱が生じる疾患です。
体が疲れたときなどに繰り返し生じます。
1週間程度で自然に治癒することが多いです。
③ 固定薬疹
お薬を内服して半日以降に口のまわりに紅斑が生じます。
ピリピリした痛みを伴うこともあります。
お薬を内服するたびに皮疹が生じ、だんだんと色素沈着が強くなっていきます。
市販の解熱鎮痛薬で生じている患者さんが多いです。
④ 口なめ病
こどもに多い疾患です。
舌で口のまわりを頻繁に舐めることによって生じます。
舌の届く範囲に境界明瞭な紅斑や色素沈着等が生じていることから診断します。
治療としては、亜鉛華単軟膏などの外用により舐める習慣をやめさせるようにします。
治った後も、日ごろのケアとして、ワセリンを塗布して保護するようにします。
⑤ カンジダ症
くちびるのカンジダ症では、くちびる全体に赤くなり、薄い皮膜を伴います。
免疫力が低下している方、ステロイドの外用を長期続けている方に生じることがあります。
その他、紫外線による口唇炎、膠原病に伴うもの、悪性腫瘍によるものなどもあります。

Cheilitis is a disease that causes inflammation of the lips. There are various causes, but the main ones are:
① Contact dermatitis
It is caused by an external irritant adhering to the lips. It is often accompanied by itching. Lip balm, lipstick, toothpaste, and cosmetics are often the culprits.
The causative agent is suspected through interviews, and its use should be discontinued. If necessary, perform a patch test to identify the cause.
As a treatment, if the redness is strong, use a steroid ointment. For daily care, apply Vaseline externally to protect the lips.
② Herpes simplex
It is a disease that causes painful, tingling vesicles on the lips. It occurs repeatedly when the body is tired. It usually heals spontaneously within a week.
③ Fixed drug eruption
Erythema occurs around the mouth half a day after taking the medicine. It may be accompanied by a sharp pain. Each time you take the medicine, you will develop a rash, and the pigmentation will gradually become stronger. Many patients develop this disease on over-the-counter antipyretic analgesics.
④ Mouth licking disease
It is a common disease in children. Caused by frequent licking around the mouth. Diagnosis is based on the presence of well-defined erythema and pigmentation within the reach of the tongue.
As a treatment, the patient should be given topical zinc oxide ointment to stop the habit of licking. After healing, apply petroleum jelly as a daily care to protect the lips.
⑤ Candidiasis
In candidiasis of the lips, the lips are red all over and accompanied by a thin film. It may occur in people with weakened immune systems or those who have been using topical steroids for a long time.
In addition, there are cheilitis caused by ultraviolet rays, those associated with collagen diseases, and those caused by malignant tumors.

口角炎（こうかくえん）
口角炎とは
口角炎とは、くちびるの端が赤くなったり、切れたりする病気の総称です。
原因としては、次のようなものがあります。
① 物理的刺激
歯ブラシや歯磨き粉、食物などによる刺激による口角炎です。
治療としては、なるべく刺激を減らし、弱めのステロイド軟膏を外用します。
② 栄養不良
ビタミンB２、ビタミンB６、ニコチン酸アミド、亜鉛、鉄などが不足することにより口角炎が生じます。
治療としては、ビタミンBや亜鉛などの内服を行います。
③ 口唇ヘルペス
ピリピリする痛みと小水疱が生じます。
治療としては、ビダラビン軟膏の外用や抗ウイルス薬の内服を行います。
繰り返すことが多いです。
④ 感染症
自身の免疫力が低下している場合などに、カンジダなどの真菌やブドウ球菌などの細菌が感染して口角炎を生じる場合があります。
また、アトピー性皮膚炎や加齢などにより皮膚自体が弱くなっていると、口角炎が生じやすくなります。

It causes tingling pain and vesicles. Treatment includes topical vidarabine ointment and oral antiviral drugs. It is often repetitive.
④ Infectious disease
If your immune system is weakened, fungi such as Candida or bacteria such as Staphylococcus can infect you and cause angular cheilitis.
Also, if the skin itself is weakened due to atopic dermatitis or aging, angular cheilitis is more likely to occur.
